Health Screening and Forms

Health Screening will begin in November for grades  K-8th. If you do not wish to have your child screened, please send a written letter to school, attention Andrea Lattanze.

Physical Examination forms were due Oct 15th. Please send in immediately to Nurse's office, any remaining forms.

Physical Examination:  Grades K, 6 & 11 (and new school enterer)

Dental:                             Grades K, 3, 7  (and new school enterer)
